Job summary

The Post holder will work collaboratively within the acute care setting as part of the Colorectal team, acting as a main point of contact for patients who have been referred on a two week wait pathway and are under the continuing care of the Clinical Nurse Specialist team. You will provide administration support to assist in ensuring two week wait patients are managed appropriately, collecting and collating information on the appropriate data base. In addition, the post holder will be expected to provide the same administration support for patients who are on the stratified pathway.

Main duties of the job

To collaborate, coordinate, and communicate with the wider healthcare team, ensuring that following a two-week wait referral, patients are booked into the appropriate clinic and/or referred for suitable diagnostic investigations.Assisting in supporting the team with the implementation of the stratified pathway and the straight-to-test process. This includes contributing to the development and delivery of streamlined patient pathways that enhance efficiency and patient experience.To assist the team with service provision and work flexibly according to the needs of the service, which can often be demanding and unpredictable. This requires adaptability and a proactive approach to managing changing priorities.To support the team in ensuring all patient appointments, investigations, and procedures are arranged within specified timescales to provide an optimum level of care for each patient.Assist the team in ensuring waiting time targets are met and not breached, while also ensuring patients' needs are addressed. This includes working alongside and providing administrative support to the Clinical Nurse Specialists.A key aspect of the role involves daily and direct communication--face-to-face and via telephone with patients, relatives, carers, and other health and social care professionals. Given the sensitive nature of cancer care, this communication requires a high degree of empathy, understanding, diplomacy, honesty, and integrity.
