Job summary

An exciting opportunity has arisen for a band 6 (indicative banding) to join our new childrens hospital at home community team service at Scunthorpe. However the post holder maybe required to work in Grimsby. The post holder must be an enthusiastic and positive individual, with a can do attitude and demonstrate experience as a paediatric community nurse.

Paediatric community nursing is forever evolving and the service requires the post holder to lead with the other members of hospital at home team and work in partnership with other multi disaplinary teams.

The service covers a 7 day service including bank holidays and operates between 8am and 8:30 Monday to Friday and 8am-6pm Saturday and Sunday

Main duties of the job

Working within the NMC Code of Professional Conduct and be responsible for ones own professional conduct and monitor these aspects with the paediatric community team

Work jointly with other team members and to influence the overall efficiency and development of paediatric community nursing from hospital to home

identify and develop clinical changes to working practice and working in different ways to enhance service provision and work with managers to achieve this

provide daily leadership and clinical support for community and service users (patients, parents/carers primary and secondary care)
