Job summary

An exciting secondment/fixed term opportunity has arisen for a Band 4 Cancer Care Coordinator across Grimsby and Scunthorpe hospitals.

The Post holder will work collaboratively within the acute care setting as part of the Colorectal team, acting as a main point of contact for patients who have been referred on a two week wait pathway and are under the continuing care of the Clinical Nurse Specialist team. You will provide administration support to assist in ensuring two week wait patients are managed appropriately, collecting and collating information on the appropriate data base.

Main duties of the job

You will possess excellent organisational skills, be able to demonstrate competency in IT and knowledge of IT systems, and have excellent communication skills. You must be flexible in your approach to working and have the ability to work well using your own initiative.

? To collaborate, co-ordinate and communicate with the wider healthcare team ensuring that following a two week wait referral patients are booked into the appropriate clinic and /or referred for appropriate diagnostic investigations.? Assisting in supporting the team with the implementation of the straight to test process.? To assist the team with the service provision, and work flexibly to the needs of the service, which can often be demanding and unpredictable? To assist the team to ensure all patient appointments, investigations and procedures are managed and arranged within the specified timescales in order to provide an optimum level of care for each patient.? Assist the team to ensure waiting time targets are not breached and patients needs are met.? To work alongside and provide administration support for the Clinical Nurse Specialists.
