Come and join our community healthcare team who provide support for patients when leaving hospital that require short term care and rehabilitation. The service will provide re-enablement and care support for patients and their families over seven days a week.
The post holder will work in collaboration with the Macmillan home care team, District Nurses, locality coordinators, discharge teams and will closely link with community therapy services to promote functional independence and rehabilitation for short term care and support needs. There may be occasions of cross cover dependent on service demands.
Access to a vehicle with business insurance is essential for this community-based role.

Our Senior Healthcare Assistant (SHCA) roles are focused on ensuring that high quality patient centred care is delivered in a safe clinical environment.

We have a dedicated Practice Development team focused on ensuring our staff have the right skills to care for our patients, this includes a weeklong induction for new starters where clinical skills are taught and practiced preparing you for your new role.
Main duties of the job
The role will involve assisting with patient's personal care and support needs up to 4 visits a day over 7-days a week.
The role will involve caring for patients who have recently been discharged from hospital and are returning home with a change in care and support needs. With compassion, the successful applicant will be expected to nurture independence where ever possible to promote improved quality of life.

- To maintain the highest possible standards of compassionate and professional patient centred care with particular attention to privacy and dignity.
- Assist patients with personal care, hygiene and sanitary needs, ensuring privacy and dignity is maintained at all times.
- Assist in meeting patients' nutritional needs, including non-oral nutritional and fluid intake. Ensure that patients who require assistance at mealtimes have an appropriately coloured tray to identify they need help and are assisted with eating and drinking.
- Where identified in the plan of care, ensure that food and fluid intake and output are accurately documented on appropriate patient charts
