An exciting opportunity has arisen for a Registered nurse to join our experienced team of Home Therapy Clinical nurse specialists . We are looking for a dynamic, highly motivated and enthusiastic nurse with an interest in peritoneal and home haemodialysis, along with a commitment to continuous learning. The successful candidate will be an integral part of the team and will provide direct, safe and effective care to patients on home dialysis.
We offer you a friendly and supportive environment with a mentor and protected supernumerary period. Previous renal experience is desirable but not essential as fully training will be provided.
Main duties of the job
- Responsible for the organisation and management of a caseload of Home dialysis patients, working autonomously to deliver specialist clinical care appropriate to the needs of the patient.
- Provide specialist training to patients and support person to enable them to safely carry out Home dialysis treatments.
- Use excellent presentation skills to deliver training and education sessions and be able to manage varying levels of understanding
- Perform clinical assessments of patients on a Home Therapy, using up-to-date knowledge and skills in the planning, implementing and evaluation of care ensuring patients safety at all times.
- Promote patient autonomy through education and support to enable individuals to make informed decisions about their care.
- Refer to other specialist professionals that the patient may require as appropriate
- Act in an advisory capacity to provide support and advise to patients, support person, nursing staff and external agencies regarding all aspects of the patients chronic kidney condition.
