Clinical Nurse Specialist - Hypertension
We are seeking a clinical nurse specialist for the hypertension service at UHNM.
This exciting role will enable the ideal candidate to work autonomously running nurse led clinics.
The role will consist assessing and planning treatment in line with National Guidelines on the treatment of hypertension
Main duties of the job
The CNS will run their own nurse led clinics, this involves the assessment and treatment planning of patients referred to the hypertension service as well as requesting investigations of possible secondary causes of hypertension.
Although the role is autonomous there is a designated renal consultant that the post holder will work closely with.
As CNS you will also liaise with colleagues in endocrinology and cardiology as well as GPs who refer their patients about test results and any further onward investigations required.
The role also includes patient education into lifestyles changes - weight loss, stopping smoking, diet and exercise and counselling patients.
You will be given the opportunity to undertake the non-medical prescribing course if you don't already have this qualification.
Knowledge of cardiology, renal and diabetic nursing is essential for the role as the age range of patients is from 16 years to the elderly.

This post will provide a comprehensive highly specialist service to clients with hypertension and needs a good understanding of kidney disease, cardiology and diabetes. They will act as an expert and resources for members of other multi-disciplinary teams.The post holder will provide specialist clinical leadership within this area of care and will lead and develop specialist educationThe post holder will act as the key worker and liaise with other healthcare professional to provide the best possible specialist care for their patientsWill plan and manage with direct responsibility complete episodes of care, undertake independent assessment and management of patients to incorporate pharmaceuticalconsiderations, making complex clinical decisions regarding patient management and clinical outcomes.Lead on delivering national standards including hypertension services specifications for hypertension patients and shared care decision making and liaison with health care professionals.To work autonomously using advanced levels of history taking and examination skills within guidelines and protocols. Identify other conditions and has ability to manage other conditions to acceptable levels whilst acknowledging limitations and appropriate referral to medical staff.Ensure health promotion, health education in smoking cessation, weight management, alcohol misuse.
