Main duties of the job
As a Band 5 Nurse, you will play a key role in providing high-quality, compassionate care to patients, working closely with colleagues across both hospital and community teams. You will support patients throughout their care journey, helping to plan and deliver care that meets their individual needs and promotes dignity, comfort, and independence.
The role includes supporting patients and their families, administering medication, monitoring wellbeing, keeping clear and accurate records, and helping to ensure care is well coordinated. You will be supported by experienced staff while continuing to develop your own skills and confidence, and you will also help guide and support students, junior colleagues, and healthcare support workers as part of the wider team.

Clinical
- Take clinical responsibility for patients within your allocated area of work
- Continuously assess, plan, implement and evaluate treatment, using appropriate programmes/methods of care
- To ensure a high delivery of quality care in accordance with Trust policies, procedures and guidelines. Participating in activities to improve the quality, productivity and effectiveness of care in response to local and national policies and initiatives
- To practice in accordance with the NMC Code of Professional conduct and scope of professional practice and maintain own NMC registration
Communication
- Showing empathy, understanding, and professionalism, when communicating with patients, their carers and families. Identifying individual needs and therefore utilising a range of agreed communication mediums
- Communicate effectively and professionally with colleagues and all levels of staff
- Maintain confidentiality at all times, as required by legislation and Trust policy
Professionalism
- Ensure records and documentation are maintained, kept confidential, and meets professional NMC guidelines, appropriate Trust standards, and in line with the Data Protection Act
