Job summary
Place Based Team - Warwick & Kenilworth strives to provide patient centred care, and actively encourage prevention and self-management, therefore the applicant will be required to work closely with the multi-disciplinary team, working collaboratively, and strengthening relationships with our GP's.
As a Clinical Support Worker you will need to be flexible and hardworking as you will be expected to see patients in their own home environment, under minimal supervision within your competencies and adhering to SWFT's Lone Working Policy.
You will have at least 1 year care experience preferably in the community and have some knowledge and skills in managing the care of long term conditions, wound and pressure ulcer care and end of life patients. Enthusiasm and excellent communication skills together with being a good team player are all attributes we are looking for.

Main duties of the job
The Band 3 Clinical Support Worker will undertake high quality, caring and compassionate personal care and associated duties under the direction but not direct supervision of the registered staff and working as part of the multidisciplinary team.
As a Band 3 Clinical Support Worker (CSW) you make a valuable and important contribution to the delivery of high quality healthcare, supporting the registered professional in the delivery of safe, effective, compassionate patient care.
