Job summary
An exciting opportunity has arisen for a dynamic, forward thinking, and self-motivated individual to join our Community Response Team based over 2 sites in Leamington and Stratford.
The Community Response Team operates across the hours of 08:30- 21:30, 7 days a week, including weekends and bank holidays. Shifts are allocated pro rata, we offer a mix of early, late and long days.
To be successful with this post, you must live in the UK and have a full NMC registration.You must also possess a valid driving license and have access to your own car for this role. Mileage will be paid for work-related travel.
As a Community Nurse working in CRT you will be responsible for assessing, identifying and managing patients needs who have had a deterioration in their health. You will have a varied caseload seeing patients with long term conditions, falls, acute illness, supporting patients at the end of their life, and providing holistic, comprehensive care to patients in their own homes. The Community Response Teams' aim is to prevent hospital admissions and facilitate hospital discharges within a timely manner. You will also work closely with the Multidisciplinary Team, including: Therapists, GPs, Social Services, Urgent Response and many more. You will be working collaboratively with the other community service teams to deliver seamless care across the locality, working within an unpredictable and unscheduled workload.
Main duties of the job
The community staff nurse is a member of the Integrated Health Team, whoundertakes professional duties in the client's homes and clinics to ensure provision of high standard nursing care and health promotion. To work across professional boundaries, providing comprehensive health care that is sensitive to the needs of the local population.
