Job summary
The Heart Failure Specialist Nursing Team (HFSN) is pleased to offer the exciting opportunity of newly created Band 3 Clinical Support Worker role to will support the service based across South Warwickshire. The Team is based at Acorn House, Warwick and provides clinics and home visits across the whole of South Warwickshire.
The service currently runs Mon - Fri 8:30 - 16:30hrs.
We are looking to recruit a competent and well organised individual, with a particular interest in heart Failure. The role requires the management of patients with a diagnosis of heart failure, and the post holder will assist in the delivery of a plan of care to meet the patient's needs with emphasis being placed on self-care.
The successful candidate will be motivated, enthusiastic and be able to demonstrate good interpersonal skills. The post is a community post and the ability to meet the travel requirements of the role is essential.
Main duties of the job
The Band 3 Clinical Support Worker will undertake high quality, caring and compassionate delegated duties under the direction but not direct supervision of the registered staff and working as part of the multi-disciplinary team.
