Anticoagulation and VTE Clinical Nurse Specialist

Job responsibilities

Key Result Areas:

To deliver a patient centred service which involves a consultation and computerised anticoagulation dosing service for a case-load of patients, both in-patients and out-patients. Work closely with the Consultant Haematologist service Lead.

To act as a role model and clinical expert for all nursing and medical staff within the Trust and the community extending advice to other healthcare professionals such as General Practitioners.

To be responsible for the on-going evaluation of the effectiveness of patient care, including evaluation of own practice and ensure that patient records are maintained in accordance with the Trust policy using paper and electronic records.

Ensure patient follow up is rigidly maintained, using computer software, liaising with administration staff and discharging to General Practitioners as necessary.

To be competent and confident with the use of Near Patient Testing Devices.

Identifying patients who require the domiciliary service, liaising with General Practitioners, phlebotomy staff and district nurses to arrange this.

The nurse specialist will liaise closely with the cardiology team; ensuring patients are adequately treated as per NICE guidelines.

The nurse specialist will liaise closely with the various medical disciplines for those patients requiring anticoagulant follow-up following venous thrombosis as per the agreed pathway.

The nurse specialist liaises with all other specialist fields in the preparation of pre-op and post-op management following policies and procedures.

Knowledge skills and experience required:

Act as a role model and clinical expert providing specialist, expert clinical knowledge to clinical colleagues, patients and carers / relatives.

Undertake clinical supervision of nursing colleagues on an individual or group basis.

Participate in the development of a strategic plan for (Specialty) and assist the Manager / General Manager to implement change.

Ensure the effective and efficient use of physical and financial resources.

Participate in the development of strategies for delivering effective specialist care within a changing environment that improves the quality of care and health outcomes in the specialist area.

Monitor health, safety and security of self and others and promote best practice in the service area.

To promote and support innovative practice and ensure relevant research findings are incorporated into practice.

To contribute to audit and research activities within the speciality and in collaboration with multi-disciplinary colleagues.

To participate in the promotion and development of the speciality by publishing and presenting innovations, audit and research findings locally and nationally.

To attend regional and national meetings on anticoagulation, such as the RAID Anticoagulation Therapy Management System.

Take responsibility for personal development and education with regard to PREP and Personal Development Plan objectives.

Participate in Clinical Supervision for self and others and utilise reflective practice techniques.

Communications and Working Relationships:

Promote team working and meet with manager and clinical lead to discuss complex cases and participate in shared learning.

To provide specialist education to members of the anticoagulation team, which facilitates learning and individual personal development

Produce suitable written educational materials for staff, patients and carers, and play an active role in their education.

To be involved in the supervision of students as necessary.

The coordination of education and training programmes for both nursing and junior medical staff.

Demonstrate empathy and compassion to patients and carers throughout their pathway of care and management. Use persuasive and advanced communication skills to deliver sensitive or complex information to reduce barriers in language and understanding.

Qualifications

Essential

• 1st level registered nurse
• Evidence of competency in teaching and assessment
• Registration with NMC or HCPC
• Recognised anticoagulation qualification

Desirable

• Relevant diploma or degree or evidence of commitment to pursue

Experience

Essential

• Minimum of 3 years' experience in a specialist role in Anticoagulation Nursing
• Evidence of ability to work autonomously
• Previous experience of formal and informal teaching of patients and staff

Desirable

• Experience in ambulatory DVT/PE management
• Evidence of developing and leading nurse led clinics/services
