Job summary
An exciting opportunity has arisen for a dynamic, motivated registered nurse to join the Anti-coagulation Service at SWFT, as an Anti-coagulation Specialist Nurse. The post encompasses all areas of anti-coagulation, and the successful applicant will be caring for patients who are at risk of or being treated for a variety of different thrombosis or emboli.
Management of patients includes the assessment, initiation, management and evaluation of patients on anticoagulation therapy (including Low molecular weight heparin, direct oral anticoagulants - DOACs and warfarin and other vitamin K antagonists). We offer a wide variety of clinics and whilst we do undertake face to face consultations, a significant number are via the telephone. We are also looking to expand inpatient support in the future. The post involves undertaking significant clerical duties. The successful applicant will have good organisation skills and be able to demonstrate the ability to work in methodically and consistently, with the ability to meet deadlines in a timely manner.
Main duties of the job
To provide expert advice and interventions within the anticoagulation service, that maintains and improves standards of care and promotes a seamless service within a multi-professional framework. This will be achieved through clinical practice, management, education, research and audit of professional activities taking into account objectives of the clinical service group, corporate objectives and within the Clinical Governance Framework. The primary role of the post will be to work in the Anticoagulation team using the Raid computerised dosing system. The nurse specialist will act as a clinical expert alongside a fellow Band 6 Nurse and provide a professional source to the multi-professional team, patients and carers within the anticoagulation service. To supervise support staff within the anticoagulant department, keeping the Manager and Clinical Led Consultant informed of any problems.
