South Warwickshire Foundation Trust - Women & Children's Division.

We are seeking highly skilled nurses with a strong clinical background in children and young people's care. The post holder must have excellent interpersonal communication and leadership skills. You must have the ability to work, autonomously and in a team and be flexible in working shifts including night duties and weekends to ensure the service is covered.

The post holder will need to be dynamic, innovative, forward thinking and be confident to sensitively challenge conventional thinking.

The post holder will work with an established award winning innovative team, across Warwickshire.

The post holder will initiate, influence, develop and deliver effective care practices in response to the child young person and their family's needs. They will be expected to also develop their skills at being competent in caring for Children and Young People who have technological needs such as requiring ventilation. This will ensure that the service that is delivered, is holistic, safe, effective, trusted and compassionate.

Main duties of the job

To promote and sustain the independence of children & young people (CYP) with a range of complex healthcare needs by delivering skilled nursing care within the community in a variety of settings.

To undertake assessments of care, develop, implement and evaluate care programmes, in partnership with the child, young person and their family. To carry out managerial responsibilities for an identified caseload and skill mix of staff.

Job description

Job responsibilities

To assess, plan and implement care to meet the identified individual health care needs of CYP, and families (this will include provision of continuing care support).

To work in partnership with other care providers to facilitate, a prompt and efficient discharge from hospital, subsequently reducing hospital admissions and unnecessary re-admissions.

Working in partnership with the family, Consultant Paediatricians, Nurse Consultants and key agencies, from both the statutory and voluntary sector.

To participate in the risk analysis and decision making process at child protection meetings, providing written reports and giving health advice where needed.

Initiate and develop flexible and innovative approaches to meet the needs of CYP and their families.

Following a period of induction to participate in telephone out of hours advise service and 24/7 support when delivering an episode of active palliative care on call.

To maintain a professional attitude at all times, developing the role within the Community Childrens Nursing Team, (CCNT) in the best interests of the profession and in accordance with the NMC Code of professional conduct. Also providing on-going evidence of eligibility to practice.

To assess and identify equipment which is suitable for CYP and family needs following the medical devices and procurement policy.

To be flexible to work within a 24 hour shift pattern, including weekends and bank holidays.

Knowledge, Skills and Experience

Participate in Induction, training and preceptorship programmes for qualified nursing staff, including student nurses, healthcare support workers and as agreed with other agencies.

To participate in relevant nursing forums to establish links.

To be actively involved in the development of standards, protocols, care pathways and clinical audit.

To be in involved in providing training to enable staff from multidisciplinary & multiagency partners to meet agreed competency levels.

To provide clinical supervision and support to staff, one to one and group supervision and carry out personal development reviews as required.

Develop own and others knowledge and practice and organisational boundaries by providing clinical advice as required.

Contribute to monitoring quality and the investigation of incidents.

Update and maintain professional knowledge in relation to your area of interest around the national and local modernisation agenda for children, young people and their families.

To be aware of own learning needs and limitations and to maintain professional expertise by embracing the concept of lifelong learning.

To work alongside the clinical lead to support the operational running of the CCNT. (e.g. Sickness return to work interviews etc).
