Job summary

We are excited to announce the Trauma & Orthopaedic department are looking for an experienced clinician to work as part of the established SWAT Team in elective and trauma orthopaedics.

We are looking for a dynamic, committed Orthopaedic Physiotherapist / Occupational Therapist / Nurse (Band 6) to support the Clinical Lead and Ward Manager in supporting a hybrid acute and community team to manage patients from admission through to their management back at home in the community.

All staff members are supported to develop clinical and leadership skills by the visible senior leadership team. The successful candidate will be someone who has previous orthopaedic experience, has a desire to drive a new service and has the ability to set the tone for high quality care. We are looking for a clinician who is able to champion safe, effective, inclusive, compassionate and trusted patient care.

Main duties of the job

The role of the Band 6 SWAT Team Nurse/Physiotherapist/Occupational Therapist is to deliver high quality, caring, and compassionate patient care. The post holder will support the SWAT Team in managing trauma & elective orthopaedic patients, providing a high standard multi-disciplinary service in both acute and community settings, including the orthopaedic wards and home environments, over a 7-day service.

This fixed-term post has been created to support the provision of late therapy to the elective (e.g. inpatient therapy shifts to 7.30pm aiding same day discharge following arthroplasty surgery).

The role will involve autonomously undertaking assessment, implementation, and evaluation of care needs for Orthopaedic patients both in the acute hospital and in the community. The post holders are required to work as part of a team committed to delivering and improving a high standard of care within South Warwickshire NHS Foundation Trust.

Clinical Practice

To gain informed consent from each patient prior to assessment and treatment. The clinician should be familiar with and adhere to South Warwickshire Foundation Trust policies on consent.

To undertake a comprehensive assessment of the individual patient both adults and children including those with diverse or complex presentations/multiple pathologies; gaining relevant information by clinical examination; observation, questioning, discussion and manual assessment techniques (such as range of motion).

Apply advanced clinical reasoning skills to formulate a clinical diagnosis of their condition and set therapeutic/rehabilitation objectives.

To formulate and deliver an individual rehabilitation treatment program based on specialist knowledge of evidence-based practice and treatment options using clinical skills e.g. manual techniques, patient education, exercise program or classes and other alternative options (such as continuous passive motion machines). Ensuring competency to practice is maintained as appropriate.

To evaluate patient progress, reassess and alter management plans as required.

To formulate a prognosis and recommend best course of interventions developing comprehensive treatments and discharge plans.

To make informed clinical decisions on timely discharge planning and / or referral to other services.

Communications and Interpersonal Relationships

To communicate the clinical diagnosis and facilitate understanding of a mutually agreed treatment programme to patients and carers.

To communicate effectively with other patients, relatives or carers on a group or individual basis in order to maximise rehabilitative potential through verbal and non- verbal methods. This may require explanation, instruction, persuasion, motivation, negotiation and reassurance or use of behavioural modification techniques.

To communicate with colleagues and other relevant professionals professionally and appropriately and in a timely manner (e.g. medical notes, letters and reports).

To assess individual patients understanding of proposed treatment modalities and have the ability to work within a legal framework with patients who lack capacity to consent to intervention, in line with Trust policy in gaining consent.
