Job responsibilities

Expertise & Excellence in Clinical Practice

An understanding and experience working within outpatient and/or asimilar setting, this must include establishing and/or delivering specificclinics.

Ability to assess, plan, implement, deliver and monitor tobaccodependency advice and support sessions throughout their outpatientjourney. This must include advice on Nicotine Replacement Therapymedications, on-going care management, and working withinrecommended protocols and guidelines provided. This may involveindividual, telephonic, on-line and group work sessions in a variety ofsettings and venues.

A sound understanding of developing individually tailored support plansincluding recommending treatments based on assessed needs.

Be able to provide evidence based general health, lifestyle advice andsupport to people using the service e.g. healthy life style choices,physical activity, and stress management. This will include individualand group-based interventions, signposting and referring to otherhealth, community, and voluntary services.

Have experience in and be competent to participate in educatingpatients about the benefits of stopping smoking, particularly prior tosurgery, and give support to those wanting to quit. Promote smoke freehomes and support and advice about passive smoking.

Undertake nursing interventions consistent with evidence-basedpractice, transferring and applying knowledge and skills to meetpatients needs.

Maintain accurate up to date and comprehensive records at all times.Furthermore, to contribute to the monitoring and evaluation of theTobacco Dependency service.

Ability to have difficult but motivational conversations with patientsabout the risks of smoking, including the importance of a smoke-freehousehold.

The above indicates the main duties of the post, which may be reviewed in thelight of experience and developments within the service. Any review will beundertaken in conjunction with the post holder.

Clinical

To practice in accordance with the NMC Code of Professional conductand scope of professional practice and maintain own NMC registration

Take clinical responsibility for patients within your allocated area ofwork.

Continuously assess, plan, implement and evaluate treatment, usingappropriate programs/methods of care and to ensure a high delivery ofquality care in accordance with Trust policies, procedures andguidelines.

Participating in activities to improve the quality, productivity andeffectiveness of care in response to local and national policies andinitiatives.

Identify and use educational strategies to deliver information topatients and carers.

Participate in the development of patient focused education includingtraining to support self-management and health promotion activities.

Assist in the development of other professionals by contributing toTrust wide specialist education and training.

Support the delivery of formal and informal teaching initiatives as partof the Trusts education strategy as agreed with the appropriate seniornurse to ensure practice development, staff empowerment andimproved care for patients.

Take personal responsibility for life-long learning and personaldevelopment through clinical supervision and appraisal. Activelyengages with learning and development opportunities needed to workas an advancing practitioner and take appropriate action to ensurethese needs are met.

Evaluate current research findings and if appropriate implement intopractice in the speciality area.

Play an active role in the development of the smoke-free service targetsand work with collaboratively with team members to share best practiceand information to achieve these.

Undertake relevant training to maintain the skills and knowledge asnecessary through participation in continuous development. TheNational Centre for Smoking Cessation Training (NCSCT) practitionertraining is essential to this role.

Respond effectively to referrals within stated timescales self-referralsfrom the individuals, and those from colleagues /other workers/GPs.

Ensure that the relevant equipment is provided and maintained on anagreed schedule. Report any failings of equipment or otherenvironmental factors to the Tobacco Dependency Nurse SpecialistLead.

To be involved in the monitoring and evaluation of client feedback,offering recommendations as part of the team to improve services.

Be aware of how to report incidents, concerns or complaints andescalate any concerns or complexities to the Tobacco DependencySpecialist Nurse.

Alert the appropriate line manager to any issues in work (includingconcerns about individuals or work in the community) and maintainconfidentiality.

Communications and Working Relationships:

Take personal responsibility for ensuring effective communicationbetween all service providers.

Act as a role model for excellent communication skills. Linked toTobacco Dependency, demonstrate empathetic interpersonal andcommunication skills in supporting, informing and advising patients andcarers through diagnosis, treatment, disease progression, prognosisand supportive and palliative care where applicable. This will involveoccasionally imparting significant news or supporting patients andcarers during and following such consultations.

Communicate effectively and professionally with colleagues and alllevels of staff.

Maintain confidentiality at all times, as required by legislation and Trustpolicy.

Effectively and sensitively communicate complex and sensitiveinformation to emotionally distressed patients and family/carers at keypoints along the care pathway on a frequent basis.

Contribute to the audit, monitoring and evaluation ofthe service, including the completion of the minimum data set (NHSEngland mandatory requirement). This will include following up patientsfollowing discharge to monitor stop smoking outcomes.

Contribute to the development of policies and clinical guidelines tosupport the specialist service.

Collaborate with other MDT members to develop and updatemultiprofessional Trust operational policies and guidelines.

Maintain patient records and ensure an effective documentation systemto reflect the activity and delivery of specialist nursing care.

Promote the service were applicable.

Supports management of change as required.

Assist with the collation of data to provide evidence of productivity,outcomes and quality through audit and research.

Participate in clinical research relevant to practice area.

Assist in the development of links and system to seek the views ofpatient and carers to ensure service development is in line with userneed

Qualifications

Essential

• Registered Nurse with NMC (Adult Nursing)
• Evidence of continuous professional development
• Obtained an educational level of GCSE level A - C English and Maths or equivalent

Desirable

• NCSCT Stop Smoking practitioner training - or willing to undertake within 3 months of stating in post.
• NCSCT Pregnancy and Smoking Cessation Training or willing to undertake within the 3 months of stating in post

Experience

Essential

• 12 months relevant nursing experience
• Experience of assessing patient needs and responding with relevant care plans
• Experience working as part of a team and/or autonomously
• Experience of using hospital related computer system and electronic records

Desirable

• Understands the need and importance of The Tobacco Dependency Service
• Experience of supporting the smoke free pathways for patients
• Experience in working with patients in the delivery of the health promotion agenda
• Experience of supporting patients who wish to give up smoking
