Physiotherapist - Permanent Contract
Nuneaton and North Locality: Community Recovery Service (CRS)
South Warwickshire Foundation Trust is committed to bringing care closer to home. A position has arisen for an enthusiastic Band 6 physiotherapist within our Community Recovery ServiceTeam - Nuneaton/North Team, North Warwickshire.

The successful candidates will work with patients in their own homes to deliver an integrated rehabilitation pathway across community services. You will liaise and work closely with Community & Specialist Nursing Colleagues, General Practitioners and the acute hospital, as well as with therapists within the Integrated Health Teams.
The Integrated Community Health Service operates a seven day service from 08:00 - 22:00 so flexible working is required. Including weekends and bank holidays. (Therapists currently work up to 6pm) The ability to travel independently across Warwickshire is Essential.

Main duties of the job
To provide a high standard physiotherapy service to patients in specialist area.
To work as an accountable and autonomous practitioner, holding responsibility for own caseload.
To work in conjunction with the Pathway and Team leads in planning, co-ordinating, delivering and evaluating the Physiotherapy service within the speciality on a day to day basis.
To take a role in the provision of support and clinical supervision of less experienced physiotherapy staff and undergraduate students.
To deputise for the Team leaders as required.
To keep accurate records of patient contact using written records and electronic data capture.
To contribute to out of hours working as required.
To maintain own competency to practise.
