An exciting opportunity has arisen for an experienced nurse to join a well-established team working within the iSPA Integrated Care Co-ordination Service (ICC).
ICC is a service which triages urgent patient referrals and monitor patient telehealth systems such as Docobo. Your role will be to triage, monitor telehealth systems, wmas admission prevention referrals, refer, allocate and support access to out of hospital services, primary and secondary care.
Clinicians will use their sound clinical knowledge and risk assessment skills to clinically triage and clearly identify urgent and non-urgent patient referrals allocating and arranging visits to our patients within the community setting.
ICC are at the forefront of technology with OOHCC and are always looking at interactive resources and systems to provide a smart and efficient service.
Main duties of the job
The post holder will use their sound clinical knowledge and risk assessment skills to prioritise referrals through the clinical triage service as necessary.
The Post holder will effectively coordinate the resources available within Out of Hospital services to maximise access to services or sign post to other appropriate services available.
The post holder will ensure that all contacts with the clinical triage services reach an effective and appropriate outcome for the patient.
Organisational
To provide managerial support to the team including Performance Development Reviews, Clinical Supervision and Recruitment and Selection.
To provide leadership to the clinical team.
To provide knowledge and guidance with respect of clinical health services and to ensure and facilitate an effective interface between primary care services and out of hospital services.
Contribute towards ensuring the AHP & Nursing Strategy is implemented.
Participate in the development, training and mentorship of pre-registration students/medical students and staff as required including induction and preceptorship programmes.
To provide assistance with the resolution of complaints within the clinical speciality, or sphere of responsibility.
