South Warwickshire University Foundation Trust has an exciting opportunity for a motivated, values driven and passionate nurse/Allied Health Professional for the role of Consultant Advanced Clinical Practitioner (ACP) in the Out of Hospital Care Collaborative.

The successful candidate will report to the ACNO with clinical supervision by the DCMO. The OoH division work flexibly with our employees, providing support and direction. We have many forums for us to meet as a senior management team. We value our staff very highly and hold monthly health and well being forums in each PLACE.

Main duties of the job

The post holder will take a major role in providing the highest level of quality expert practice for patients who are suitable for rapid response services and admission avoidance new models of care and support discharge pathways This involves acting as a catalyst for change in practice and service development, challenging professional and organisational boundaries and generating new solutions that best meet the needs of patients and their families. The post holder will work both collaboratively and autonomously, with authority, using advanced knowledge and skills, to help develop and deliver services across organisational, professional and geographical boundaries through a networked approach. The post holder will work at an expert level across the 5 recognised domains of the Consultant role, these being:

  • Expert practice
  • Strategic and enabling leadership
  • Learning, developing an improving across the system
  • Research and innovation
  • Consultancy

The Consultant ACP will have significant clinical experience and a proven record of leadership, change management and innovation. The role will encompass expert clinical nursing care along with key responsibilities for service developments, education and training, professional leadership and consultancy.

Expert Practice:

The post holder will have a pivotal role in the ongoing development and implementation of best evidence-based practice in relation to the delivery of new models of care through strategic development, direct expert clinical work, education and training and audit, research and evaluation.

To have advanced theoretical and practical knowledge of a range of work procedures and practices across a number of specialist fields.

To provide consultancy as an expert practitioner demonstrating advanced clinical skills and knowledge associated with the speciality.

To provide leadership in the training of all staff in providing highly complex and sensitive information e.g. breaking bad news and complex clinical matters.

To provide ongoing clinical training and support for nurses and therapists in rapid response, new models of care, admission avoidance pathways.

Build on existing links with local universities and the Trusts Training and Development Department.

To provide clinical supervision, formally assessing other staff, identifying training and development needs, and developing and implementing strategies to facilitate knowledge and skills development across professional and organisational boundaries.

To work in partnership with education providers to ensure continuing professional development provision meets local needs, participating in the development of new courses as required.
