Job summary
We have an exciting opportunity for experienced nurses to join the Place Based Teams in Rugby.
The post holderswill be requiredto supporta busy workloadwithin the urgent and unplanned work flow,deliveringhigh quality, holistic nursing care to patientsin their own home.
We are looking forcompetent and organised clinicians withexperience of caring for patients withcomplex careneeds andlong term conditions. Preventionof hospital admission,where appropriate,andsupportingend of life patients in their own homes are key elements of this varied and rewarding role.
The successful candidateswill be motivated and enthusiastic,and must be able to demonstratean innovative and flexible approach to service delivery.
In return the applicant will be supported by a dedicated, passionate and experienced team, with regular clinical supervision and relevant training.
The post is based at The Orchard Centre, Rugby.
Applicant musthold a full driving licence and have access to ownvehicle (Car Lease scheme in operation).
Main duties of the job
The community staff nurse is a member of the Integrated Health Team, who undertakes professional duties in the client's homes and clinics to ensure provision of high standard nursing care and health promotion. To work across professional boundaries, providing comprehensive health care that is sensitive to the needs of the local population.
