Job summary
We are looking for a motivated and exceptional individual to join the Patient Safety team on a 12 month secondment. The Quality Lead position involves complex analysis of incidents to identify learning for the Trust. It is an exciting but challenging time to join the team, as we are mid preparation to implement the new Patient Safety Incident Response Framework (PSIRF) in the Autumn of 2023. This will replace the current national Serious Incident Framework. We have established processes currently in place; the successful candidate will be at the forefront of introducing new ways of working moving away from in-depth written reports for every incident to more proactive and timely reviews and identification of learning. There will still be a requirement at times to produce complex, accurate and fair written reports, other learning methods will be utilised and you will need to be adaptable to this. There will be challenges but this makes this opportunity even more exciting!
You will need to be proactive, thorough and unbiased and able to communicate exceptionally well with staff, families, senior management and external stakeholders. This is a pressurised role - time management skills and the ability to prioritise your workload are essential. Main duties of the job
Support the Patient Safety Manager and Head of Clinical Governance in the management of the Trust's Review, Learn, Improve (RLI) incidents and ensure that the Trust meets statutory and national obligations and to contribute to continued improved service delivery.
Be an expert in RLI incident investigation, techniques and practice; embrace new ways of working with the advent of the Patient Safety Incident Response Framework (PSIRF).
Be able to employ suitable investigation techniques applicable to incidents of concern or complaint.
Undertake formal incident investigations, producing high quality investigation reports and presenting findings to internal review meetings, recognising that PSIRF will bring new ways of working and requirements.Undertake Coroner and clinical negligence claim reviews, presenting findings to Inquests or Court if required.
Provide expert advice to Trust committees, groups, managers, staff and legal representatives on Patient Safety and Operational quality issues.
Provide expert advice and support to appointed Investigating Officers responsible for patient safety incidents and serious complaints, drawing on experience.
Prepare and deliver training to staff and managers on Patient Safety.
Liaise with other Trust departments to ensure learning points from investigations are developed into Trust practice.
Attend relevant Trust and external meetings to provide learning points from investigations/reviews and develop Trust practice.
