Stroke Specialist Nurse

Job summary
The Stroke Specialist Nurse (SSN) will work across 7 days on a rotational basis, as a key member of the Integrated Community Stroke Service Team to ensure all patients in a defined area who have had a stroke, or TIA have access to specialist assessment and associated care.
Service Aims:
o Improve management and assessment of risk factors for patients with stroke/TIA
o Improvement of long-term care for patients with stroke/TIA management
o Early identification and management of stroke related complications
o Reduce acute admissions
o Improving access to other services
o Providing advice and signposting to counselling
The SSN will monitor progress, provide follow up and risk assessment, recognise new symptoms related to stroke (and refer to specialties as necessary) and continue to provide ongoing advice and support in relation to stroke, and TIA.
The successful candidate will be supported by our Practice Educator for Stroke and the ICSS Stroke Team.

Main duties of the job
You will be responsible for the assessment, planning, implementation, and evaluation of care for stroke and TIA patients, without direct supervision, to patients, carers and relatives in a variety of environmentsincluding, home, hospital, clinic, virtual.To work in partnership with patients and carers to offer support and advice and promote informed choice.To support the role of the stroke locality Team leader and ICSS Team members.To work autonomously, providing effective, evidence-based stroke care in line with proficiencies and training record and organisational policies, procedures and guidelines.To support the ICSS team by working in a flexible way.
