Job summary
Are you an experienced community nurse looking to develop your skills in long term conditions management? Do you strive to provide holistic care for people with complex health needs?
We are looking for a proactive, motivated and skilled nurse in long term conditions management to join our small team in Penwith consisting of 3 community matrons, 2 long term condition community nurses and a healthcare support worker. You will support patients in the community with complex health needs to help reduce admissions to hospital and will work as part of an integrated care team. You will be the keyworker for patients who require additional support and management of their Long Term Conditions.
The working hours are 28.5 hours over 3 days, flexible working will be considered. At present there is no weekend or bank holiday working.
To apply for the post, you will need to be a registered nurse with previous community experience . You will either need to have undertaken or working towards a post graduate certificate in long term conditions.
This post is a great opportunity to develop your skills in complex nursing management. You will be supported through regular supervision and opportunities to undertake study.

Main duties of the job
This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly. o Cornwall Foundation Trust o Royal Cornwall Hospitals NHS Trust o Cornwall and Isles of Scilly Integrated Care Board If you do not currently work for one of these NHS organisations, your application will not be considered at this time. As an autonomous practitioner you will be responsible for the assessment and treatment of patients with complex health and social care needs ensuring provision of a holistic nursing assessment and high quality, safe, evidence based care to patients and carers with the aim of maintaining patients in their own homes.
You will work collaboratively, managing patients with chronic disease and frailty through autonomous practice including taking managed clinical risks with the ability to discuss or escalate e.g. where the patient is kept at home or in a care environment, rather than hospital admittance as would previously been the practice.
This is an adaptable and evolving role, focusing on safe, effective admissionavoidance and supporting timely discharge from acute hospitals, with a whole system approach. You'll co-ordinate care and strive to reduce the risk of acute re-admissions by identifying risks,implementing management plans and collaborating with the whole community resource to facilitateongoing care management.
