Specialist Dietitian Stroke

Job summary

The lead specialist dietician will work across 7 days on a rotational basis, across the county as required, as a fully integrated member of the multi-disciplinary integrated community stroke service team(community).

To work as a specialist autonomous practitioner providing advanced clinical expert dietetic support and advice to patients and Health Professionals in the specialist area of stroke care.

To support the band 6 dietician and more junior members of the ICSS team and provide leadership in the development of dietetic care as part of the multidisciplinary team.

To lead and contribute to policy development on matters relating to service development for the ICSS Team.

To develop appropriate resources within this specialist area of dietetic practice.

To support education and training across the stroke pathway and to act as a clinical resource for colleagues.

To help to establish Research and Development and a robust Audit review of service within this specialist area of dietetics.

Main duties of the job

Service Aims

Support safe discharges from hospital and provide a high standard rehabilitation service to patients referred to the Integrated Community Stroke Service

Provide specialist assessment, effective treatment, and individualised goal setting of patients in their own home/ place of residence, to promote recovery post stroke.

Early identification and management of stroke related complications.

Reduce readmissions due to complications post stroke.

Provide education and support to formal and informal carers.

Promote the development of self management skills following stroke.

Provide advice and signposting to appropriate services

Qualifications/ Education

Essential

  • Degree in Nutrition/Dietetics or equivalent leading to registration with Health and Care Professions Council (HCPC)
  • A teaching qualification or significant experience of teaching groups.
  • Specialist training (or relevant clinical experience) in stroke care
  • Post graduate qualification at masters level (or awaiting confirmation of their final grade) or relevant equivalent experience gained through substantial advanced clinical practice
  • Comprehensive experience of enteral feeding in the hospital and community setting.
  • Range of experience of other conditions that impact on the nutritional needs of a patient e.g. renal, gastroenterological, other neurological conditions,

Desirable

  • Membership of the British Dietetic Association (BDA
  • Advanced communication skills

  • An ability to demonstrate best dietetic practice for a range of conditions
  • An ability to communicate with a wide range of people and using differing media
  • An ability to evaluate and report on complex data
