An exciting opportunity has arisen at Camborne Redruth Community Hospital for a Progress Co-Ordinator to join the Roseware SRU Team. You will play an important role in supporting the clinical staff in facilitating safe and timely discharge from hospital for patients.
You will help provide a seamless service for patients which will include cross boundary and collaborative working with the multi-disciplinary team along with colleagues in Adult Social Care and other outside agencies in the community setting.
This is an ideal opportunity for an enthusiastic and dynamic individual. You will need to have good interpersonal skills, IT skills and possess excellent communication and organisational skills.

This is a part-time post to include occasional weekends and bank holiday on a rota basis. Shift split over the week to be discussed at interview and agreed with successful applicant.
Main duties of the job
- The Progress Co-ordinator will be based at the Rosewarne SRU based at Camborne Redruth Community Hospital. The inpatient ward has 20 beds.
- To assist with the efficient day to day running of a busy ward, providing administrative and clerical support to the nursing, therapy and medical staff providing a 7 day service. This post will be required to work some flexible hours to meet the requirements of the post, eg evening meetings with carers and participation in the Progress Co-ordinators weekend rota.
- To ensure that patient's needs are addressed, based on the care plan and co-ordinated in the most timely and effective way.
- To act as a link, liaising with other agencies and departments, to provide efficient and effective communication to expedite progress towards the chosen home environment.
- Patients are received from Acute Stroke Unit at RCHT and are discharged to a variety of places including home and residential settings.
