Job summary
An exciting opportunity has arisen within the small, vibrant community cardiac nursing team in East Cornwall. This role involves holding a caseload of complex cardiac patients, liaising with primary and secondary care and supporting cardiac rehab exercise groups.
Ideally, you will have a cardiology background to include heart failure management and cardiac rehab experience.
The successful candidate will need to travel throughout East Cornwall (subject to change to service demands) and work both autonomously and as part of this busy team.
Main duties of the job
The main duties of this role will include: symptom management and optimisation of complex heart failure patients; secondary prevention management; education and multi-disciplinary liaisons; advising primary care and regular attendance of MDTs. This will include nurse led clinics, home visits and telephone assessments and participating and supporting the cardiac rehab exercise groups.
