Job summary
We have a rare opportunity to join our North and East Integrated Transfer of Care Hub (iToCH) as an Administrator, supporting the hub in processing referrals into North and East community services.
The Administrator will play a key role in supporting patient flow by reviewing and identifying blockages, highlighting key issues to Team Managers, and ensuring accurate and timely feedback is provided to referrers. The role also involves liaising with multiple agencies to support the effective coordination of patient care.
The successful candidate will have excellent communication and interpersonal skills, the ability to work under pressure, and a strong commitment to working collaboratively as part of our Neighbourhood team.
This is a full-time position, covering shifts between 8am and 6pm, 7 days a week, including bank holidays, shared on a rota basis with other administrators.

Main duties of the job

Monitor referrals on the single electronic referral system on a continual basis, triaging in partnership with the multi-professional team.

Review and identify key blockages to patient flow, escalating and highlighting issues to Team Managers as appropriate.

Maintain accurate, clear, and timely feedback to referrers.

Liaise with multi-agency partners to support the effective coordination of patient flow.
