Are you passionate about improving care for people living with frailty? Do you have a strong background in clinical programme management and education delivery? We are seeking an experienced and dynamic Programme Manager to lead the development and implementation of the Southwest Frailty Learning Hub. This is a unique opportunity to shape a regional resource that will enhance frailty awareness, improve workforce capability, and support better outcomes for individuals living with frailty across the Southwest.

You will lead the creation of a centralised Frailty Learning Hub, bringing together system-wide training offers and resources. The hub will support both registered and non-registered workforce across acute, community, primary care, social care, VCSE, and care home settings. Working collaboratively with regional partners, including ICSs and SWAST, you will ensure the hub reflects the diversity of the Southwest's coastal, rural, and urban communities.

Main duties of the job

The Programme Manager will be responsible for coordinating the development of the vision, strategy and associated programmes of work related to the development of the Southwest Frailty Learning Hub. Lead the creation and delivery of a centralised Frailty Learning Hub, bringing together system-wide training offers and resources. The hub will support both registered and non-registered workforce across acute, community, primary care, social care, VCSE, and care home settings.

They are responsible for coordinating the design, development and delivery of this complex programme in line with the delivery of system and regional objectives, to successfully deliver multiple, concurrent, independent and interdependent programmes of work to enable the delivery of the regional Hub.

The Programme Manager takes a lead on understanding and interpreting national direction for Frailty including policy initiatives, as well as regional change initiatives to steer the strategy of the service, in line with national, regional and system objectives. They work with stakeholders across the southwest region to drive the direction of the programme by coordinating and collaborating with system and regional partners including commissioning, voluntary, community and social enterprise, and other providers.

They are responsible for leading the development and delivery of a programme of work through the full governance framework from scoping and obtaining the organisation mandate through to evaluation.
