Job summary

An exciting opportunity has arisen for enthusiastic Community Nurses to join our Mental Health Rehabilitation Service. Based at Bodmin Hospital, the team currently operates from Engineers House at St Austell Hospital. We provide countywide support for individuals recovering from complex and enduring mental health conditions, working alongside Fettle House inpatient facility to deliver a seamless rehabilitation pathway into independent living.

Our team collaborates closely with carers, families, housing providers, education, pharmacy, and other specialists such as the Early Intervention Team. We also assist with discharges from acute wards. The role involves person-centred care planning, supervising support workers, and contributing to service development. Regular supervision and support are provided.

This is a full-time post with shifts between 08:30-20:38, seven days a week. Whether you're local or relocating, you'll enjoy Cornwall's stunning surroundings while making a real difference. Main duties of the job

Working as a team member, offering specialist and generic skills. You will devise and implement interventions that enhance engagement with services and support clients and their carers to obtain the treatment and support that promotes and maintains wellbeing and promotes recovery.

Develop holistic care plans that will include positive risk management, and all areas relevant for the client care.

Show good Assessment, formulation and evaluation to care planning and evidence that clients and their carers are encouraged to actively engage in the process, and showing an understanding of cultural and diversity barriers to receiving relevant care and ways of overcoming these.

High standard of communication, able to liaise with members of different services and disciplines using the whole team approach, be able to support clients in presenting their needs/requests in a clear and effective manner.

Be able to engage with clients in a manner that promotes and maintains ongoing adherence to care packages.

On occasions manage the day to day running of the team and visits county wide.

  • Current professional registration - NMC
  • Professional qualification - RMN,
  • To act as a mentor for student university placements. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required

  • Previous experience within housing employment services
  • Previous work with community/ Acute services

Knowledge and abilities
