Developmental Consultant Nurse - End of Life Care

Job summary

The post holder will be working as a Advanced Clinical Practitioner to develop into a Consultant Nurse and undertake a bespoke training/development plan to achieve the higher-level of performance across the four pillars of advancing practice expected from our Consultant Practitioners.

Cornwall Partnership NHS Foundation Trust is creating a new vision for Community and Palliative End of Life Care across the county. Our aim is to ensure that people nearing the end of life experience a calm, peaceful, and dignified death in the place of their choice.

Currently, Cornwall has one of the highest rates in the country for people spending extended time in hospital during their last 90 days of life. This highlights the need for a high-quality, patient-centred service based in the community, allowing more people to receive the care they need at home.

We are working to build a service that meets individual needs, reduces hospital stays and unnecessary interventions, and offers a single point of access and personalised care plans to support home-based care.

In partnership with the Macmillan Social Investment Fund and local organisations, we are expanding and strengthening current services. This includes a dedicated 24/7 Rapid Community Response and Care Service, as well as a round-the-clock phone line for the public and professionals focused on Palliative and End of Life Care at home.

Main duties of the job

Managing patients with SPEOLC needs with malignant and non-malignant diseasesand multi-morbidity through advanced level autonomous practice including takingmanaged risks. For example assessing people coming into palliative virtual careservice, monitoring disease process, assessing deterioration and prescribing withinscope of practice.

Have clinical responsibility for the palliative virtual patients and work collaboratively with all professionals, carers and relatives to gain a deep understanding of all aspects of the patient's physical, emotional and social situation, optimisingsymptom control, psychosocial wellbeing, advance care planning and end of lifecare.

Facilitating preferred place of care and preferred place of death.

To work with the palliative consultant team and the Community Specialist PalliativeCare Nursing (CSPCN) Team to provide advice service to support healthcareprofessionals seeking advice to support their care of patients with SPEOL needs
