Job summary
Are you passionate about making a real difference in people's lives? This is an exciting opportunity for an energetic and enthusiastic individual to join our Dementia & Older People Mental Health (DOPMH) team, supporting individuals with a dementia diagnosis or a functional mental health illness.
As a Healthcare Assistant (HCA), you will work closely with our multi-disciplinary team (MDT), which includes a Consultant Psychiatrist, Community Psychiatric Nurses (CPNs), Memory Assessment Nurse, Occupational Therapist, Dementia Liaison Nurse, Psychologist, and the Primary Care Dementia Team.
What we're looking for:- A caring, compassionate approach and excellent communication skills.
- Ability to work as part of a team and independently in the community.
- Previous experience in healthcare or mental health support is desirable but not essential - full training will be provided.
Main duties of the job
What you'll do:
- Support patients and their carers in the community, either in their own home or in a care setting.
- Deliver time-specific, care-planned contact alongside the patient's care coordinator.
- Provide a range of services, including practical and emotional support, as outlined in individual care plans.
- Work collaboratively with the MDT to ensure high-quality, person-centred care
