The HomeFirst Restormel team is part of the Central HomeFirst area. It is a multidisciplinary team comprising registered and non-registered nursing, therapy and administrative staff. We work collaboratively with partner organisations to support the intermediate care needs of our local population.
This position is open to physiotherapists and occupational therapists with experience of working in the community and who wish to develop their assessment, care planning and reablement skills whilst being part of a dynamic, integrated, multiagency, multidisciplinary team.
You will be responsible for assessment, planning and implementation of care and assist in the interpretation of information relevant to the patient's condition/progress, identify problems and needs and set realistic goals. You will need excellent organisational skills along with a wide knowledge base and experience of working in older person's services.
We will offer a working environment in which to develop your clinical and interpersonal skills, a structured in-service training programme; appraisal and supervision system.
We are based in St Austell and cover the Restormel area including Newquay, Padstow, Wadebridge, Mevagissey, and surrounding areas.
Main duties of the job

You will need to be able to a triage, assess and provide evidence based treatment plans for adults within the catchment area, either in their own homes or nursing and care homes.
To work alongside the wider multidisciplinary teams within the community, liaising with the community and acute hospitals, Adult Social Care and the Voluntary Services.
