Are you a community nurse experienced in assessing patients in their own homes? Do you have knowledge of palliative and end-of-life care, with a real passion for improving quality of life? We want to hear from you.

We're looking to grow a small team of Band 5 Palliative Care Support Nurses across Cornwall. In this role, you'll work alongside the Specialist Palliative Care team, taking responsibility for assessment, care planning, implementation, and evaluation under the guidance of a Specialist Palliative Care Nurse.

Your role will involve supporting patients, carers, and families in their home environment, helping them maintain independence and achieve the best possible quality of life.

You'll work via our staff bank, to cover temporary shortages in SPC caseloads due to sickness or staff changes. This position is a great opportunity to develop specialist skills and gain insight into the Specialist Palliative Care Nurse role.

Main duties of the job

Responsible for the assessment, planning, implementation and evaluation of care, under the remote management of a Specialist Palliative Care Nurse, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.

To work in partnership with patients and carers to offer support and advice and promote informed choice.

Sponsorship is not available for this position as it does not meet the UKVI eligibility criteria for a skilled worker visa. Any applications from individuals who require sponsorship to work in the UK will therefore be declined.
