Acute Care At Home Team Leader

Job summary

The opportunity has arisen to lead the community based Acute care at home team for the North and East locality of Cornwall. This role offers an exciting opportunity to lead a team of senior nurses providing support and treatment to people in their own homes. The Acute care at home team deliver treatments which include the administration of IV medications and the monitoring of people during an acute episode of illness to enable patients to recover at home safely. The team will support early discharge from the acute settings, whilst also providing care to prevent a hospital admission where possible. The team work closely with the Virtual wards and Hospital at Home teams to identify people requiring this care. The team leader will be responsible for the professional and operational leadership, for quality and governance within the team ensuring the service is cost effective and efficient whilst also seeking opportunities for team and service development.

Main duties of the job

Keyelementsoftheroleare:daytodaycoordinationand supervisionoftheteam, providing complex casemanagementandsupport for the team. To be an autonomous practitioner responsible for the assessment and provision of treatments for patients on the caseload. To manage the performance, and clinical effectiveness of the team ensuring the delivery of evidence based care. To be able to provide assurance that performance standards are being met whilst meeting service targets. To identify new opportunities for service development.
