Job summary
Are you a Pharmacy Technician seeking a rewarding and dynamic role where you can contribute to patient safety and work as part of a multidisciplinary team across both mental health community crisis services and hospitals? You'll provide clinical pharmacy services to the Initial Response Service across Cornwall. Your base location is negotiable, though travel to other sites will be required. Working patterns, including a rolling weekend rota, are flexible and open to discussion for the right candidate.
This role will require working independently and collaboratively within the wider pharmacy team. As a Medicines Optimisation Pharmacy Technician within the crisis service, you'll support medicines reconciliation, prescription reviews, discharge facilitation, ward-based dispensing, medication recycling, patient counselling, and audits.

To provide a comprehensive medicines management service to patients with the Initial Response Service and support Mental Health hospital inpatients which includes medicines reconciliation, prescription review and endorsement, recycling of medication, supporting self-medication and, where necessary, patient and carer counselling.
To participate in multi-disciplinary activities such as attending ward handover meetings and facilitation and preparation of discharge medication including the handover of information regarding a patient's medication to other healthcare professionals if required.
To work with the pharmacy team in delivering clinical pharmacy and safe systems of working with medicines across mental health sites, with scope to work with patients directly in community.
To supervise and support with education and training activities such as student technicians, student pharmacists and pre-registration pharmacists
This post will support hospitals within Cornwall. Base location can be negotiated but there will be a requirement to travel to other peripheral hospitals.
