Job summary
We have an exciting opportunity for a District Nurse Clinical Lead to join our caring and highly motivated Tamar Valley and Launceston Community nursing teams in the area of East Cornwall.
This is a key role, within our service to support our service to achieve the trust vision of "Delivering high quality care" in their day to day work.
Within a geographically aligned Primary Care Area this role will be clinically responsible for the diagnosing, prescribing, planning and evaluation of care, ensuring provision of high standards of care to patients and their families within the community. They will work as part of Integrated Teams, working within and across professional and organisational boundaries.
A key element of this role will include, the monitoring of the quality of care provision and identify and lead on areas of service development.
We are looking for a dynamic, flexible and compassionate practitioner who can demonstrate competencies and is able to adapt to a wide variety of clinical circumstances.

Main duties of the job
The post holder will monitor the quality of care provision and identify and lead on areas of service development.
They will be a registered nurse who holds or working towards the DNSPQ qualification. They will work within an integrated team providing clinical leadership for all patients within the community nursing caseload through proactive management. Whilst working on their own initiative they will be responsible for ensuring that all members of the nursing team are providing the highest standard of evidence based clinical care and will actively support and liaise with other health and social agencies and professionals.
