We have opportunities available across various localities within the county, offering flexibility in base location depending on service need and candidate preference with a requirement to cover Cornwall and the Isles of Scillys.

Main duties of the job

The role of the progress co-ordinator is a countywide role working with Clinical Prioritisers, Palliativecare team (Acute Hospital and Community Hospitals) and Hospital at home. The role supports the Endof Life Pathway processing and managing the flow of patient referrals through the STRATA.

The progress co-ordinator will monitor the referrals on the single electronic referral system on a continualbasis, triaging in partnership with the multi professional team.The progress co-ordinator will be responsible for reviewing key blockages to patient flow, highlightingkey issues to ward colleagues and Patient flow team by maintaining accurate and timely feedback toreferrers, liaising with multi-agencies for the co-ordination of patient flow.
