We have opportunities available across various localities within the county, offering flexibility in base location depending on service need and candidate preference with a requirement to cover Cornwall and the Isles of Scillys.

Main duties of the job

To assist in the delivery of clinical care, planned and assessed by the Registered HCPs.

To carry out care / other duties under indirect supervision of a Registered HCP.

Support care on a day-to-day basis for all patients in the clinical area in a manner sensitive to the individual clients needs and wishes, always acknowledging their rights and beliefs.

Contribute to resource management within the clinical area.

The post holder will rotationally work within the Telephone response service for Palliative and End of Life Care providing a 24/7 working closely with admission avoidance services to ensure patients safety is maintained.

You will be required to collect any referral information and with support from the clinical team assist in directing the caller/ referrer to the correct service for their immediate requirements.
