Job summary

Cornwall Partnership NHS Foundation Trust is creating a new vision for Community and Palliative End of Life Care across the county. Our aim is to ensure that people nearing the end of life experience a calm, peaceful, and dignified death in the place of their choice.

Currently, Cornwall has one of the highest rates in the country for people spending extended time in hospital during their last 90 days of life. This highlights the need for a high-quality, patient-centred service based in the community, allowing more people to receive the care they need at home.

We are working to build a service that meets individual needs, reduces hospital stays and unnecessary interventions, and offers a single point of access and personalised care plans to support home-based care.

In partnership with the Macmillan Social Investment Fund and local organisations, we are expanding and strengthening current services. This includes a dedicated 24/7 Rapid Community Response and Care Service, as well as a round-the-clock phone line for the public and professionals focused on Palliative and End of Life Care at home.

These new developments will complement our existing Hospital@Home virtual service and a new partnership with the voluntary sector to enhance community-based End of Life support.

Main duties of the job

The post holder will work in the specialist of palliative and end of life care and be responsible for practicing autonomously in line with Health Education England's standards for consultant level practice. The post holder will provide expert practice in palliative and end of life care, strategic and enabling leadership, promoting learning, development and improvement and research and innovation, both in the palliative care teams and across the Trust. The consultant nurse leads the palliative care clinical nurse specialists and is accountable to the Chief Nurse.
