Job summary
Cornwall has designed a collaborative to listen and respond to care homes which has been created to deliver safer, kinder, and enhanced person-centred care to people that live in care homes with improved equality and integration. It is part of a wider transformation program supported by Cornwall's Integrated Care System and funded through the Ageing Well Program.
The Clinical Specialist will work across Cornwall, being responsible for the delivery of a program of support and development aligned to care homes within each of the three Integrated Care Areas (Central, West, and North/East). They will develop an interface with care homes at its heart and collaborating with the Local Authority, Primary Care Networks, Community Services and wider health and social care partnership.
This is a varied autonomous role with clinical assessment, research opportunities, quality improvement and training and education at its core. We are advocates for the residents and the care staff helping them to navigate through primary care.
Key to this role is the completion of urgent Community response (UCR) assessments for residents suffering an acute deterioration requiring an urgent response.
