Job summary
The Falmouth & Penryn PCN are looking for a mental health professional to join our mental health and wellbeing team. The team has been together for 3 years and work with the local GPs to provide a patient centred trauma informed service to the population. We are looking for someone who is passionate in supporting individuals to find wellbeing in their lives, someone who is flexible and keen to work closely with the team, open to changes and development within the service and interested in a variety of approaches to support our patient group.
The post holder will be responsible for working with the clinical lead and band 6 Mental health professional to provide clinical leadership to the team. The postholder will work with a range of individuals including those with complex and risk presentations. The team provide a "bridge" between the primary care mental health team and secondary care and will generally be people whose needs cannot be met by IAPT services but who may not need ongoing care from secondary care community mental health services.
They will work alongside general practice teams within the Falmouth & Penryn PCN, providing clinical assistance and advice to the GPs and other members of the primary care team in managing the mental wellbeing of the practice population. The primary care team includes a band 7 Clinical lead, band 6 Mental health professional (this role will be a second band 6) band 5 health & wellbeing practitioners, and a social prescriber.
Main duties of the job
The post holder will be based and embedded within a Primary Care Network (PCN) working with GPs, practice nurses and other Additional Roles Reimbursement Scheme (ARRS) workers e.g., social prescribers, health and wellbeing coaches, care co-ordinators, as part of the primary care Multidisciplinary Team (MDT). They will work with primary care GPs to manage the mental health needs of their population at the pointof contact. This includes those with lower-level support requirements that can be met within primary care, working with the Primary Care.
The role will act as interface between primary care and wider
mental health services and: -
- To provide rapid and responsive assessment, signposting and brief treatment to individuals referred to the service by several routes including self-referrals and General Practitioners.
- To provide support to Primary Care staff in the local management of patients with common mental health problems.
- To ensure that people are quickly and efficiently supported to access the most appropriate treatment for their mental health difficulty.
This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
