Job summary
We are seeking to recruit an experienced community support worker who has worked with people experiencing psychosis and other mental health conditions.
This role will include supporting individuals to improve their mental health by working with them, their families, members of the multidisciplinary team and wider support agencies within the community.
Main duties of the job
Establish and maintain effect contact with service users, their relatives and carers in order to ensure good communication.
Give information and assistance to service users and their carers so that they are aware of their rights, entitlements from the team and wider health and social care community.
Work with service users and their carers to access identified services. Monitor mental health and be aware of fluctuations in mental health through observations and report changes to the team practitioners so that service users are directed to appropriate services and risk is managed.
Encourage social/physical activities as appropriate to the serviceneeds.
Maintain up to date client records in a timely and accurate manner.
