This is an exciting role to work within the Home First Central Area, supporting the continued development of the reablement service. You will be responsible for supporting the Team Lead with the day-to-day line management of the Community Support Workers (CSWs).

The successful candidate will be an integral link between the CSWs, Assessors, Team Lead, and wider stakeholders for the facilitation of great patient care. This will be predominantly office based but will require visiting patients in their own home at times.

Main duties of the job

The post holder will support the Team Lead and registered Assessors to manage the Community Support Workers (CSWs) working mainly within the geographical of Carrick in Cornwall but could mean supporting colleagues elsewhere in the Central Cornwall area. The post holder needs to be able to support co-ordination of referrals from a variety of Health and Social Care Professionals, allocate visits, communicate and answer queries from all disciplines including care providers, members of the public and users of the service. This involves organising own day to day activities, planning of both straightforward and more complex ongoing referrals, together with looking into the longer-term planning of care.The post holder will be support the Team Lead with the leadership and management of the CSWs. Daily meetings will be held to review patient care and act on any red flags identified. The role will include supporting the Team Lead with the regular supervision and annual appraisals for the CSWs, and ensuring training, audit and lone worker compliance are up to date.The Carrick Home First team supports patients who have been recently discharged from hospital as well as those who are in crisis in the community to avoid hospital admission. There will be times where you will need to visit patients in their own homes so clinical skills are essential.
