Job summary
This is an exciting role which involves working with GPs, partner organisations, secondary care, community teams and community hospitals to support the development of existing services regarding the management of patients with complex and advanced care needs closer to their homes or in the immediate community setting. The post holder will provide training and support clinical and complex skill delivery such as Tracheotomy, TIVAD, and Gastronomy management and Blood Transfusion. Will develop training programmes to support and empower team/s in the care of highly complex patients in the community setting. Ensure appropriate education is developed, delivered and evaluated in relation to complex clinical skills.

Main duties of the job
Responsible for the day to day facilitation of patients requiring complex clinical skills, providing on site clinical skills support and training to appropriate team/s related to complex clinical skills.Take an active role ensuring staff are safe in clinical practice, work with team manager to risk assess areas of practice, reporting to senior staff with education/training plans to aid the mitigation of risk as required.Be proactive in identifying, putting forward proposals and carrying out research in the clinical environment.This role has exciting opportunities that involve
Work alongside team manager to support staff in clinical techniques unfamiliar to them.Risk assess and analyse complex clinical situations to ensure team manager is supported and given the correct advise when supporting staff to deliver complex care to patients with highly complex needs.Support team during the management of complex tracheotomies, central lines, cannulation, intravenous therapy administration, blood transfusion, enteral/parenteral feed management etc.Identify, analyse and interpret complex situations and use professional judgement across a broad range of situations.
