Job summary
We are looking for a dynamic, passionate, and motivated nurse to be a District Nurse Clinical Lead within the Falmouth/Penryn Community Nursing Team. You will be a registered nurse who holds the District Nurse Specialist Practitioner Qualification or is currently working towards it.
As well as being an exceptional role model, the post holder will work with the team to ensure provision of a holistic nursing assessment and high quality, safe, evidence-based care to patients and carers with the aim of maintaining patients in their own homes. Our patients are at the centre of all that we do so we are looking for someone who is a calm, caring, compassionate clinician with an interest in delivering the highest quality care and to support our patients to lead full and independent lives whilst promoting self-management of their health conditions.
Community Nursing leadership has a defined career progression pathway that can be clinical or operational. This post is newly created and offers the team senior and specialist clinical leadership. As an exciting new job role within the locality it gives the opportunity to explore multi-disciplinary ways of integrated working. Flexibility and a willingness to explore different ways of working is a must and be adaptable accordingly, supporting others in the change process. You will be supported fully by the District Nurse Team Manager within the role.
We provide a service between the hours of 08:00-22:00, 7 days a week.

Main duties of the job
The postholder will monitor the quality of care provision and identify and lead on areas of service development.The postholder will be a registered nurse and hold the DNSPQ qualification or be working towards this.They will be providing clinical leadership for all patients within the community nursing caseload through proactive management. They will focus on ways to achieve and demonstrate overall improvements in health care. They will work collaboratively with all members of the multidisciplinary team to establish a whole systems approach to the management of patients accessing healthcare in the area.The postholder will support their peers and be an effective team member. Whilst working on their own initiative, they will be responsible for ensuring that all members of the nursing team are providing the highest standard of evidenced based clinical care. They will have a commitment to collaborative working, and will actively support and liase with other health and social agencies and professionals.The post holder will be geographically aligned to Primary Care Networks and be clinically responsible for the diagnosing, prescribing, planning and evaluation of care, ensuring provision of high standards of care to patients and their families within the community. They will work as part of Integrated Teams, working within and across professional and organisational boundaries.
