Job summary

Successful candidates will be invited to an Assessment Day on Friday 15th December 2023 held at Carew House in Bodmin. Please note this will be either a morning or afternoon session to be confirmed.

We are looking for an experienced, caring motivated Community Nurse to join our Community Nursing Team.

You should possess commitment to team working and innovative delivery of excellent standards of care.

You will need to be able to demonstrate a real enthusiasm for Community Nursing as well as a desire for ongoing professional development.

This a permanent part time position within the Bodmin & 3 Harbours Community Nursing Team area working out of Bodmin or Par Health Offices

The ability to travel independently across the area to fulfil the requirements of the post is essential

Full and part time hours available working on a rota system covering 7 days a week.

Flexible shifts patterns of evenings between 1700 - 2200 will be considered and can be discussed in interview.

Main duties of the job

Responsible for the assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.To work in partnership with patients and carers to offer support and advice and promote informed choice.To support the role of the Community Nurse Team leader.To work autonomously, providing effective, evidence based care in line with competency and training record and organisational policies, procedures and guidelines.To support all locality teams by working in a flexible way. Moving to work with the team in most need on a daily, weekly or monthly basis as service provision demands.
