Job summary

The Stroke Specialist Speech and Language Therapist in Integrated Community Stroke Service will work across 7 days on a rotational basis, as part of the ICSS Team to ensure all patients in a defined area who have had a stroke have access to specialist assessment and associated care. There may be a need to provide countywide cover on occasion.

Main duties of the job

Service aims

  • Support safe discharges from hospital and provide a high standard rehabilitation service to patients referred to the Integrated Community Stroke Service
  • Provide specialist assessment, effective treatment, and individualised goal setting of patients in their own home/ place of residence, to promote recovery post stroke.
  • Early identification and management of stroke related complications.
  • Reduce readmissions due to complications post stroke.
  • Provide education and support to formal and informal carers.
  • Promote the development of self management skills following stroke.
  • Provide advice and signposting to appropriate services

Qualifications and Education

Essential

  • Degree in Speech and Language Therapy (or equivalent qualification).
  • Registration with Health Professions Council
  • Evidence of own professional development and have additional specialist training and advanced professional qualifications in neurodevelopmental conditions.
  • Dysphagia qualification

Desirable

  • Membership of Royal College of Speech and Language Therapists.
  • Membership of appropriate clinical interest groups
  • Completion of short relevant courses e.g., Cervical auscultation

Experience and skills

Essential

  • Minimum of 18 months post qualification experience
  • Fieldwork educator / Clinical placement supervisor /Experience as a clinical educator of SLT students
  • Have knowledge of national and local clinical guidelines in specialty areas, and an ability to apply these to clinical practice and service policies
  • Experience of working with Augmentative and Alternative Communication
  • Comprehensive experience of working with adult patients in a variety of settings
  • Experience of working with multi-disciplinary and multi-agency teams, with an understanding of team roles and professional roles.
  • Specialist level assessment skills
  • As an autonomous practitioner to be able to deal with complex clinical issues
  • Skilled in time management and organisational skills, working flexibly either autonomously or as part of a team with the ability to engage and network professionally
  • Knowledge of national agendas influencing clinical practice eg NICE

  • Experience of working with patients post stroke in both hospital and community settings.
  • Experience of evaluating service delivery and monitoring effectiveness and outcomes of intervention, and of undertaking clinical audit Experience of participation in audit and research projects
  • Experience of managing the risks associated with unsupervised / lone working
  • Experience working in an NHS setting in the following clinical areas; Stroke, Eldercare, Neurology
  • Completion of Stroke Competencies or willingness to undertake
