Job summary
This is an exciting opportunity to be involved in a new programme of work, that utilises technology to enable individuals to remain in their own home. Virtual Care and NHS@Home provision are instrumental in enabling people to receive hospital level care in their normal place of residence, the benefits of which are very significant to the individual and the wider health system.
The Virtual Care function is to utilise remote monitoring opportunities to enable people to stay in their homes whilst receiving daily observation and condition status review by a team working remotely
The service works closely with Royal Cornwall Hospital , Community Matrons, UCR, Specialist Nurses from Cardiac and Respiratory services, Practice Nurses and GPs with prevention and early detection of deterioration at the forefront of our intervention.
Main duties of the job
The Virtual Care team intervenes when appropriate if readings trigger parameters being breached in order to maintain patients clinical safety in their own homes and therefore preventing avoidable hospital admissions.
A key part of our intervention pathway is the provision of Health education coachingto enable patients to understand their unique biomarkers which will enable them to manage their health independently when they are discharged from our service .
Key to this role is supporting the embedding of Virtual Care/ Wards into existing community services.
