Job summary

The Integrated Falls & Bone Health Service is a proactive community service in Wandsworth, which aims to reduce the incidence of falls and fractures in the older adult population.

The team comprises of Physiotherapists, Occupational Therapists, Exercise Facilitators, and Admin Support.

The team is based mainly at St John's Therapy Centre and Queen Marys Hospital and can offer home or community location based, outpatient sessions and group classes to patients in the Wandsworth locality.

We are looking for an experienced physiotherapist to join the service, whether you are a band 6 ready to progress, or a band 7 looking for a change of direction. Are you passionate about supporting individuals independence in the community? Are you keen to work as part of a multi-disciplinary team assisting service users to achieve their goals? Main duties of the job

To ensure person centred care for people who use our services and to be responsive to their needs and those of the population we serve.

Carry a clinical caseload and work as a specialist autonomous practitioner within standards of professional practice.

To ensure that own practice and services provided are consistent with available evidence and adhere to standards of practice as outlined in relevant policies.

Be responsible for collecting data related to safe practice, activity, waiting times and outcomes and to ensure that these contribute to national audits.

To identify opportunities for change and drive quality improvement in the areas relevant to the role of the service.

To work with others inside and external to the organisation to fulfil the responsibilities of the role.

To ensure good governance and risk management of services provided in association with clinical lead.

To support others to develop the knowledge and skills to be able to make every contact count in their work with people living with osteoporosis, those at risk of falls and people living with frailty and long-term conditions.
