Job summary
The main purpose of this role is to support delivery of the South West London Hepatitis Network's elimination and liver-health initiatives through accurate data management, pathway coordination and development of systems that help track patients across services. The post holder will work with clinical, data and peer-support teams to ensure patients are identified, followed up and supported through care pathways.
Main duties of the job
- Collection and reporting of treatment and activity data for STHepNet West to NHS England and the ICB.
- Management of HCV, HBV and liver-surveillance data to support engagement and treatment.
- Collection of data from Emergency Department opt-out BBV testing across South West London.
- Coordination of individual, person-centred pathways of care, ensuring communication and information flow between clinical, administrative and peer-support teams.
- Coordination of MDT meetings, ensuring information is available to support clinical decisions.
- Administrative support for coordination of the Community Liver Health Check programme.
- Identification of people or groups who would benefit from hepatitis testing or liver-health assessment.
- Working with community partners to promote and support testing and scanning in outreach settings
- Supporting the STHepNet West team with ED opt-out BBV testing and navigation into care.
