Job summary

The post-holder will support the delivery of safe and effective patient services by facilitating the provision of the most recent clinically-effective research-based tissue viability advice to Trust staff. A key aspect of the role will be monitoring the effectiveness of tissue viability interventions, as well as developing patient care-pathways/Trust-wide clinical guidelines/policies with colleagues across the group. Specific focus will be on pressure ulcer prevention and management; care planning and staff support may be part of this role. The post holder will rollout education for all disciplines of staff working with patients with tissue viability complications. A collaborative approach is essential working within the multi-disciplinary team to provide comprehensive patient focused care. The post holder should demonstrate an awareness of local and national nursing agendas.

Main duties of the job

To provide advice for nursing and other colleagues regarding the treatment of patients with tissue viability complications of pressure ulceration; leg ulcers and wound healing, using the agreed protocols and working in partnership with all members of the MDT that form the Tissue Viability Steering Group.

Provide support, advice and information to referred patients and their carers regarding their tissue viability problems; treatments and services available.

To ensure all cases of Trust acquired pressure ulcers reported as category 3, 4, unstageable and DTI are assessed and investigated in line with current patient safety incident response framework guidelines.

To provide senior nurse leadership to the nursing staff of Tissue Viability Service.

Drive and co-ordinate the Trust's governance process around acquired pressure ulcers, liaising with Divisional Governance Leads and the safety Team when necessary

Continually analyse data relating to pressure ulcers, determine trends and act as a Trust wide change agent and innovator, planning, instigating, and evaluating change.

Development of policies, procedures, protocols and guidelines for treatment of patients at risk of tissue damage or deterioration.

To establish effective lines of communication within the multidisciplinaryteam, to ensure appropriate patient care, safe practice and a culture of sharing.

Take line management responsibility for the tissue viability nursing team.
