Job summary

We are looking for an experienced ACP, who is enthusiastic about delivering outstanding care to patients referred to the acute medical unit, a 58 bedded unit that caters for all referred medically unwell patients.

Patients begin initial treatment within the AAA/SDEC, a 5 trolleys area with 11 chairs that sees a variety of patients with undifferentiated diagnoses and/or on an ambulatory pathways who are discharged or triaged to a ward.

The AAA/SDEC also includes the Acute Medicine Clinic Monday to Friday.The Ambulatory Assessment Area has developed into Same Day Emergency Care (SDEC) unit and will receive patients from a variety of sources including ED, GP, 111 and LAS referrals directly.

You will be leading a team of Nurse Practitioners in conjunction with another ACP, the matrons and Medical Team.

You will be expected to work a variable rota / shifts pattern (7:30-20:00/ 8-20:30 / 9-21:30 subject to change) to ensure nursing cover on the unit 7 days a week.

Main duties of the job

  • The post holder will be practicing autonomously as an Advanced Practitioner (AP) to provide patient-centred quality care.
  • This will encompass the skills of advanced assessment, examination, diagnosis and management within an agreed scope of practice.
  • The AP will meet the requirements of the Centre for Advancing Practice's framework and guidelines for advanced level practice.
  • To work within the Multi-professional framework for advanced clinical practice in England (HEE 2017).
  • The post holder will deliver an 80% clinical component to their role and 20% related to leadership and management, education and research.
